Where it comes from
Commonly traced to a bartender at a transatlantic flying boat terminal who added whiskey to coffee for cold, delayed passengers, though the story has been retold with different details often enough to be hard to pin down.
What cooks argue about
The cream is poured over the back of a spoon so it floats as a distinct layer meant to be sipped through, not stirred in; whipped too loose it sinks, too stiff it clumps.
