Where it comes from
It spread as a lunch sandwich because cured ham keeps without refrigeration and needs no cooking, letting it travel in a lunch pail or get assembled in under a minute.
What cooks argue about
The debate is over toasting or grilling the sandwich to melt the cheese, against building it cold, which keeps the bread from going soggy but leaves the cheese firm.
